Treat Unwanted Spider and Varicose Veins!

Dear Dr. Winder:
Q.I can't bear the thought of wearing shorts this coming summer because my legs are covered with varicose and spider veins. I have one varicose vein that bulges almost the entire length of my leg; when I'm on my legs all day it causes a lot of pain. I also have several veins that I think would be considered spider veins. Is there something, that's not too expensive, that can be done to help me?

A. Almost all unwanted veins of the legs can be treated in one manner, or another. Of course, I'd need to examine you to know what might be appropriate for you; however, I'm more than happy to tell you about the different possibilities. Varicose veins "bulge" and look "rope-like"; they can cause discomfort as you've reported and are, therefore, sometimes covered by health insurance . . . and spider veins are usually found in "clusters" and look "lace-like". Varicose veins can be treated with laser (very expensive and not available in this area), by surgically cutting them out, or by a method called "sclerotherapy" (by injection). Spider veins are usually treated by sclerotherapy, as well.

When veins are surgically excised, mul-tiple, tiny incisions are made along the course of the vein, and the vein is then removed. Sclerotherapy, however, is usually preferred by patients because it doesn't leave these small scars.

Sclerotherapy involves injecting the vein with a medication called Polidocanol; this causes an inflammatory reaction to the lining of the vein, causing it to "shut down". Although not 100% effective, Polidocanol is more successful than the injection of saline solution and the most widely used method. Upon examination, I can usually determine if it is reasonable to attempt this. Patients who have it done are very pleased with the results, and feel that the fee is very affordable.
